PRINCIPAL ACCOUNTABILITIES
Bear Physical Therapy Sports Injury Center, Inc.

POSITION SUMMARY:
The Physical Therapist provides patient centered outpatient treatment to include the development of an individualized plan of care, treating patients through all phases of physical therapy, emphasizing compassionate, high quality care. Treatment is to be a thorough comprehensive and customized physical therapy regimen; ensuring they are accomplished consistent with GBPT policy and procedures. The Physical Therapist will supervise and assign patient treatment to the Physical Therapist Assistant as appropriate along with duties and performance of Physical Therapy Technicians. Will support the Clinic Manager and perform other duties as assigned.
QUANTIFIABLE DIMENSIONS:
Provides the day-to-day comprehensive evaluations of a patient and their plan of care, providing appropriate manual therapy and exercise progressions and delegate to a qualified Physical Therapist Assistant and Physical Therapy Technician, consistent with established policies and procedures. Review and comprehends budget, policies, procedures, and delivery of personal, clinical operations excellence via reporting system to the standards of GBPT. In addition, use of Strive Hub and Standardized Outcome measures (FOTO specifically) to provide data points for goal setting and reflection of the service provided.
Monitor schedule for new patients on appropriate PT schedule for evaluations.
Provide patient treatment according to referral and visit volume demands.
Monitor all patient discharges in alignment with Company standards of care.
Daily monitoring of patient visits, and cancellations.
Ensure every patient receives a thorough, comprehensive, and customized physical therapy regimen, while receiving a unique experience.
Provide a written and hands on evaluation and immediate treatment of incoming patient conditions to determine appropriate physical treatment programs.
Ongoing scheduling of patients to ensure fluid treatment progression.
Develop and implement Physical Therapy treatment programs based on each patient's particular needs.
Assist patients with all phases of Physical Therapy treatment and techniques to include:
Exercise instruction (both in-clinic and take-home instruction)
Manual therapy techniques
Modalities
Activities of daily living
Functional/total body progressions
Discuss treatment with patients and record patient conditions and responses to treatments in patient’s medical record after each visit.
Complete daily patient care notes, progress notes, discharge notes and other required forms as requested by treating physicians, rehab nurses or insurance companies.
Ensure daily treatment notes for all patients are complete prior to end of day.
Provide continuous communication with referring physicians and Physical Therapy Assistants regarding reevaluation findings to continually modify and update treatment programs.
Operate and apply knowledge of standard physical therapy equipment.
Accurately document treatments rendered in the appropriate patient’s medical record.
Ensure billing amounts on daily notes match treatment time and that all units of treatment are accounted for.
Ensure billing is accurate and complete prior to signing off on the daily note in accordance with Company policy, state practice act, and federal regulations, including but not limited to HIPAA, Medicare and Medicaid.
Review and co-sign notes of all patients treated by auxiliary personnel under PT’s direct supervision.
Ensure all patients are discharged with a positive outcome and experience or have been proactively referred to the next step to address limitations and/or complaints originally presented.
Participate in weekly/monthly meetings with CM to review production reports.
The ability to maintain friendly, cordial relations with all clients and employees; maintain a positive work atmosphere by acting and communicating in a manner that results in a positive work relationship with patients, co-workers and managers.
This position is also expected to see patients as required to meet the scheduling requirements of Golden
Perform other duties as assigned by management.
Aide supervision by company policies and procedures.

Education and Experience

Education:
Bachelor’s degree in health-related field
Graduate/Doctorate Degree in Physical Therapy from an accredited program.

Experience: Not required but considered. Appropriate state licensure, certification or registration (per state requirements), current and in good standing. *Current PT licensure or eligibility to test for PT license required.
Physical Requirements and Working Conditions
Requires prolonged standing, some sitting, bending, stooping and stretching. Some lifting, pushing and assisting patients with exercise and patient transfers. Requires eye-hand coordination and manual dexterity sufficient to operate a keyboard, mouse, photocopier, telephone, calculator and other office and clinical equipment. Requires normal range of hearing and eyesight to record, prepare and present reports. Travel
is not required.
