Job Description

Maintains a level of personal conduct in working with people supported and staff members which will promote a friendly, comfortable, safe environment where people supported may progress socially, emotionally, and physically.

Respects and promotes the individual rights of people supported including maintaining the dignity of each person during daily clinical procedures.

Identifies different communication styles to develop effective communication with staff, parents, teachers, and community partners.

Develops pro-active problem-solving skills.

Identifies concerns related to Momentum and constructively discuss concerns with appropriate leaders (supervisors, coordinators, director)

Represents Speech department and Momentum at community or events.

Participates in monthly Speech meetings: department and supervision meetings.

Acts as a liaison between the funder (school team, family, etc.) and the leadership team to pro-actively identify areas of concerns and maintain strong relationships with the community.

Maintains clinic documentation in a timely fashion, including daily notes in Raintree, and progress reports (vary by funder for SEIS, THIPArivate pay, RCs)

Manages individual schedule on Raintree, including scheduling and cancellations.

Tracks upcoming reports due based on the funder, e.g.: IEP or IFSP meetings

Maintains active state licensure and national licensure.

Provides high quality evaluations related to speech and language; develop goals as needed.

Provides treatment appropriate to the needs of the child and family; based on goals.

Maintains ongoing communication with the family and team members, to provide progress and home activities.

Collaborates with other service providers, including, when appropriate, other therapists and classroom teacher.

Participates in co-treatments with other professionals, as needed.

Attends IEP and IFSP meetings

Cleans and maintains treatment space, toys, equipment, and other treatment supplies.

Signs up for clinic space/ treatment rooms; if possible

Uses proper procedures in checking items out of the equipment lending library.

Knows, follows, and upholds Momentum’s policies and procedures.

Performs other duties as assigned to meet department and/or business needs.

Qualifications:

0-1 years of recent experience

Masters level graduate of an American speech and language association approved facility.

Collaborate with other disciplines to ensure effective and efficient patient care

SNF Experience preferred

BLS, CPR certification

Active state license

Demonstrate the highest level of professionalism when interacting with patients, families, physicians, and hospital staff members.
